Carers Assessment

When we first speak with you, we will ensure that you are happy to be registered with us. We will have a conversation with you to understand your situation, and how we might be able to help you.

You must be an adult providing care and support to an adult (18 years +) who is living in the local area served by Southampton City Council who is disabled, ill or elderly. 

What is the purpose of the assessment?
Through our conversation with you we aim to understand your caring role and how it may be impacting on your life. We focus on your physical, mental, and emotional wellbeing and identify what could help to make your life easier.

What happens during the assessment?

The assessment is a simple conversation, where you are encouraged to relax, and talk about what you feel comfortable with sharing. This is a chance for you to talk about your experience of being a carer, and perhaps for the first time for you to realise that you are truly valued for the support you provide.
The conversation is laid out in a series of questions, to help build a clear picture of how your caring role fits into your life.
Remember, you are in control of this conversation. You only share what you feel comfortable with; we are there to support you throughout.

What will the assessor ask me?


We will ask you about different areas of your life, and how your caring role may be affecting your daily life, eg
• How your relationship with your relatives or friends may be different now?
• Feelings and or emotions are you experiencing?
• Has being a carer changed the way you view yourself?
• What support you may need to help you continue in your caring role.

How long does it take?


The conversation can take up to 60 minutes and is normally undertaken by telephone, but if that does not work for you, we will try and accommodate you as best we can.

What happens after the assessment?


After we complete the conversation, we will also discuss a plan to see if things can be improved (even in a small way) for the coming year. This plan includes thoughts and ideas that may help you. If
We may ask your permission to share some of your relevant details with a third party, such as with Citizens Advice Southampton or Southampton Voluntary Services, who may be able to help you in a more practical way. We will send you a support plan with information and advice that will seek to improve your wellbeing.
We offer free advice, guidance, and engagement, to ensure that you know where to go when you need help. We are your one-stop-shop for any caring concerns.
